Terms & Conditions

By submitting any form of payment to Midwest ADP, or by scheduling services with Midwest ADP,
I understand and accept the following terms and conditions:

• I consent to receive communications from Midwest ADP via email, text message, fax, phone, and/or postal mail. These communications may include confidential information regarding my classes, SATOP programs, court case, or probation information. I understand that I may revoke this permission in writing at any time. Until I do so, I acknowledge that all communications sent by Midwest ADP are considered received and accepted.
• I understand that fees are refundable only before services are provided. If I schedule a class and fail to attend without providing at least 48 hours' verbal or written notice, I may not be eligible for a refund and may be required to pay all or part of the class fee before rescheduling.
• If I attend a class but am unsuccessfully discharged or fail to complete the class for any reason, I understand that my refund eligibility is limited. For SATOP CIP/SROP programs, any refund will equal the difference between the amount paid and the value of services already provided. For all other classes, I may not be eligible for a refund and may need to pay additional fees before rescheduling.
• Midwest ADP is certified by the Missouri Department of Mental Health to provide Intensive Outpatient Treatment (IOT), Outpatient Treatment, SATOP Screening (OMU), Offender Education Program (OEP), Weekend Intervention Program (WIP), Clinical Intervention Program (CIP), and Serious and Repeat Offender Program (SROP). Clinical staff are registered, certified, or licensed through the Missouri Credentialing Board.
• I understand that Midwest ADP provides services under Missouri Department of Mental Health and SATOP certification standards only and makes no claim that its services comply with the regulations or certification requirements of any other state.
